  • One reading is not a diagnosis. Alkaline phosphatase moves with the liver and the bones roughly equally, plus pregnancy, growth, and having eaten recently. A clinician reads it against your history and usually against a repeat, not on its own.
  • Reference ranges differ between laboratories. The bands above follow commonly reported reference ranges. Your own report prints the range its analyser was calibrated against, and that printed range is the one to compare against — a difference of a few units between labs is normal and does not mean either is wrong.
  • Alkaline phosphatase comes from two places, and the test alone cannot say which. Roughly half is liver — specifically the bile ducts — and half is bone. A raised result therefore splits into a biliary question and a bone question, and the tie-breaker is GGT on the same sample: raised GGT points at the liver, normal GGT with a raised ALP points at bone.
  • Children and teenagers run far higher than adults, and that is normal. Growing bone produces alkaline phosphatase, so the reference range through a growth spurt can reach three times the adult limit. Applying an adult range to a 13-year-old produces an alarming result from an entirely healthy child, and it is one of the more common reasons for an unnecessary referral.
  • Fasting matters more than people expect. Some people, particularly those with blood groups B and O, produce an intestinal form after a fatty meal, which can raise the result by a quarter. A borderline value on a non-fasting sample is worth repeating fasting before anything else is done.
