  • Your laboratory's printed upper limit is probably higher than the one used here, and that is the point. Most labs still report normal up to about 40–56 U/L, figures derived decades ago from donor populations that included many people with undiagnosed fatty liver. The 2017 American College of Gastroenterology guideline set the true upper limit at 25 U/L for women. A result of 85 U/L can therefore come back unflagged and still sit above the level the specialist guideline considers normal.
  • ALT is more specific to the liver than AST. AST also comes from muscle and red cells, so a hard workout, a fall, or a muscle disorder raises AST without the liver being involved. The ratio between them carries information: AST above twice ALT points towards alcohol, while ALT above AST is the usual pattern in fatty liver and viral hepatitis.
  • Fatty liver is the common cause of a mildly raised ALT, and it is often the only sign. Weight, waist circumference, triglycerides, HDL and blood sugar are the results to read alongside it, because the same metabolic picture drives all of them.
