  • One reading is not a diagnosis. Total bilirubin moves with fasting, illness, dehydration and exercise as well as liver and red-cell turnover. A clinician reads it against your history and usually against a repeat, not on its own.
  • Reference ranges differ between laboratories. The bands above follow commonly reported adult reference ranges. Your own report prints the range its analyser was calibrated against, and that printed range is the one to compare against — a difference of a few units between labs is normal and does not mean either is wrong.
  • Gilbert's syndrome explains a large share of isolated mild elevations. It affects perhaps 5% of people, raises unconjugated bilirubin only, leaves every other liver test normal, and is entirely benign. It becomes more obvious during fasting, illness or after exercise — which is exactly when routine blood tests tend to get taken.
  • The split matters more than the total. A raised unconjugated fraction points at red-cell breakdown or Gilbert's; a raised conjugated fraction points at the liver or a blocked bile duct. A total on its own cannot distinguish them, and the follow-up test is the split.
