- One reading is not a diagnosis. Cortisol moves with the hour of day above all — it peaks shortly after waking and falls to a trough around midnight — plus stress, illness, pregnancy and steroid medicines. A clinician reads it against your history and usually against a repeat, not on its own.
- Reference ranges differ between laboratories. The bands above follow commonly reported reference ranges for the time of day. Your own report prints the range its analyser was calibrated against, and that printed range is the one to compare against — a difference of a few units between labs is normal and does not mean either is wrong.
- The time of the draw is part of the result. Cortisol follows a daily rhythm: it peaks 30 to 45 minutes after waking and falls to a trough near midnight. A value of 15 µg/dL is unremarkable at 8 am and distinctly high at 4 pm. A cortisol reported without the time it was taken is close to uninterpretable, which is why blood is normally drawn between 7 and 9 am.
- A single measurement rarely settles anything. Suspected excess is investigated with an overnight dexamethasone suppression test, late-night salivary cortisol, or a 24-hour urine collection; suspected deficiency with a morning value plus an ACTH stimulation test. The single blood level is a screening step, not an answer.
- Steroid medicines change the picture completely. Prednisolone, dexamethasone and even potent inhaled or topical steroids suppress the body's own production, so a low result in someone taking them says what would be expected rather than revealing disease. Biotin supplements also interfere with several cortisol assays.
