- One reading is not a diagnosis. ESR moves with any inflammation, but also age, sex, anaemia, pregnancy and simply how much protein is in the blood. A clinician reads it against your history and usually against a repeat, not on its own.
- Reference ranges differ between laboratories. The bands above follow the age-adjusted Miller formula. Your own report prints the range its analyser was calibrated against, and that printed range is the one to compare against — a difference of a few units between labs is normal and does not mean either is wrong.
- The upper limit rises with age, and most lab reports do not adjust for it. The widely used Miller formula puts it at age ÷ 2 for men and (age + 10) ÷ 2 for women — so 30 mm/hr at 50. A printed range of "0–20" flags a healthy 80-year-old as abnormal and misses a genuinely raised result in a 20-year-old.
- ESR is slow and non-specific — that is what it is for. It rises over days and falls over weeks, so it tracks whether something chronic is settling. CRP moves within hours and answers a different question. Where both are ordered, a raised ESR with a normal CRP usually means something long-running rather than something acute.
- A raised ESR names no disease. Infection, autoimmune disease, malignancy, kidney disease, pregnancy and anaemia all raise it, and so does simply being older. It is a reason to look, not a finding in itself.
