- One reading is not a diagnosis. LDL cholesterol moves with diet, weight, thyroid function, some medicines and, to a large degree, inherited factors. A clinician reads it against your history and usually against a repeat, not on its own.
- Reference ranges differ between laboratories. The bands above follow the NCEP ATP III bands. Your own report prints the range its analyser was calibrated against, and that printed range is the one to compare against — a difference of a few units between labs is normal and does not mean either is wrong.
- These bands are population descriptions, not your target. Current practice sets an LDL goal from overall cardiovascular risk — age, blood pressure, smoking, diabetes, family history and any established disease — not from the LDL number alone. Someone who has had a heart attack may be aimed well under 70 mg/dL with an LDL of 134; someone at low risk with the same number may need nothing.
- An LDL above 190 mg/dL is treated as a finding in its own right, because at that level familial hypercholesterolaemia becomes likely enough to look for. It is inherited, affects roughly 1 in 250 people, and is the one lipid result that should prompt testing relatives rather than only treating the individual.
- Most labs calculate LDL rather than measure it. The Friedewald equation subtracts HDL and a fifth of triglycerides from total cholesterol, and it becomes unreliable when triglycerides run above about 400 mg/dL or when the sample was not fasting. If the triglyceride figure on the same report is high, ask whether the LDL was direct or calculated.
