- One reading is not a diagnosis. Uric acid moves with kidney function, diuretics, alcohol — beer especially — fructose, red meat and shellfish, and a large inherited component. A clinician reads it against your history and usually against a repeat, not on its own.
- Reference ranges differ between laboratories. The bands above follow commonly reported adult reference ranges and the gout treatment target. Your own report prints the range its analyser was calibrated against, and that printed range is the one to compare against — a difference of a few units between labs is normal and does not mean either is wrong.
- 6.8 mg/dL is where the chemistry changes. Above roughly that concentration urate exceeds its solubility in body fluid and can crystallise, which is why gout treatment aims below 6.0 rather than merely inside the reference range. A result of 6.5 is "normal" on the lab report and still above the target for someone who has had an attack.
- A normal level during an attack does not rule out gout. Urate often falls at the onset of a flare, so a test taken then can mislead. The definitive test is fluid from the joint examined for crystals; blood urate is for monitoring treatment, not for diagnosing the attack in front of you.
- Raised urate without symptoms is usually not treated. Most people with asymptomatic hyperuricaemia never develop gout, and guidelines do not recommend treating the number alone.
