- One reading is not a diagnosis. White blood cell count moves with infection, stress, exercise, smoking, pregnancy and steroids, sometimes within hours. A clinician reads it against your history and usually against a repeat, not on its own.
- Reference ranges differ between laboratories. The bands above follow commonly reported adult reference ranges. Your own report prints the range its analyser was calibrated against, and that printed range is the one to compare against — a difference of a few units between labs is normal and does not mean either is wrong.
- The differential matters more than the total. A count of 12 driven by neutrophils suggests something different from the same count driven by lymphocytes or eosinophils, and the breakdown is printed on the same report. A total on its own is close to uninterpretable.
- Benign ethnic neutropenia is common and is not illness. A persistently low neutrophil count with no infections, seen in a substantial share of people of African, Middle Eastern and West Indian ancestry, reflects where the cells sit rather than how many exist. It has been a repeated cause of unnecessary haematology referrals and of people being wrongly excluded from clinical trials.
- Ordinary things move the count within hours. Physical exertion, a large meal, stress, smoking, pregnancy and corticosteroids all raise it — steroids markedly and within a day — without any infection present.
